In the stress imprinting prompt, I saw some references to burning, stitching, electrocuting, etc. Can any of those remain or will Patho-Gen be doing literally everything possible to wipe all evidence of what they've done? It's up to the player how much remains at the end of the stay, but it's very likely there would be physical evidence of what they'd gone through. While Patho-Gen would avoid releasing anyone who looked like they got completely chewed up, limbs awol, etc., they can't heal everything. Characters would be cleaned off- they wouldn't be sent out covered in blood, or with wounds uncovered- but they could be very much still wounded.

Do they have an explanation for why 'Augmenter maintenance' includes this amount of damage...? Not exactly. But it would be brushed aside as a side-effect of the surgery. The process of altering the Augmenter is a very delicate thing, and characters' bodies sometimes responded harshly to the experience. There was a bit of ferality unleashed while they were switching out components- and to keep a character from hurting themselves or others, they had to go to some lengths to restrain them. It was necessary.
